PALB2(Partner and Localizer of BRCA2)基因编码的蛋白质是BRCA2的重要伴侣蛋白,主要功能是帮助BRCA2定位到细胞核内并稳定其结构,从而在DNA损伤修复(特别是同源重组修复)中发挥关键作用。PALB2与BRCA2结合后,能促进RAD51在DNA断裂位点的加载,这对维持基因组稳定性至关重要。PALB2蛋白的N端与BRCA1相互作用,C端与BRCA2结合,形成BRCA1-PALB2-BRCA2复合物,协调双链断裂修复。PALB2突变(如截短突变或错义突变)会导致其功能丧失,显著增加乳腺癌、胰腺癌、卵巢癌等遗传性癌症风险,尤其与家族性乳腺癌的关联性较强(约2-4%的家族性病例由PALB2突变引起)。杂合突变携带者乳腺癌风险升高5-9倍,且男性乳腺癌风险也增加。PALB2过表达可能通过过度激活DNA修复通路导致化疗耐药性,而表达降低则会引起基因组不稳定、染色体异常和细胞凋亡增加。PALB2属于FA/BRCA通路基因家族,该家族成员(如BRCA1、BRCA2、FANCD2等)均参与DNA损伤应答和同源重组修复,其共性是通过多蛋白复合物维持基因组完整性。值得注意的是,PALB2突变表型与BRCA2突变相似但通常较轻,且对PARP抑制剂(如奥拉帕尼)敏感。此外,PALB2在减数分裂中也起作用,突变可能导致生育障碍。目前针对PALB2相关癌症的筛查建议与BRCA1/2类似,包括更早开始乳腺MRI监测和考虑预防性手术。
