LBP(脂多糖结合蛋白)是一种由肝脏合成的急性期反应蛋白,属于模式识别受体家族中的一员,主要功能是识别并结合细菌脂多糖(LPS),形成LBP-LPS复合物,进而将LPS传递给CD14或TLR4/MD-2受体复合物,激活先天免疫反应。LBP在炎症和感染过程中发挥关键作用,能够放大机体对革兰氏阴性菌感染的免疫应答。LBP基因位于人类20号染色体(20q11.23-q12),其表达受炎症因子(如IL-6、TNF-α)调控。LBP基因突变可能导致其功能异常,如结合LPS能力下降,从而削弱机体对感染的防御能力,增加败血症风险。LBP过表达可能加剧炎症反应,与多种炎症性疾病(如败血症、类风湿性关节炎、炎症性肠病)相关;而LBP表达降低则可能导致免疫应答不足,增加感染风险。LBP属于脂质转运蛋白(LT)基因家族,该家族成员通常具有脂质结合和转运功能,参与脂质代谢和免疫调节。LBP与家族成员如杀菌/通透性增加蛋白(BPI)具有相似结构,但功能不同:BPI具有杀菌作用,而LBP主要起免疫调节作用。LBP在临床上有潜在应用价值,其水平可作为感染和炎症的生物标志物,并可能成为治疗靶点。
