EREG(Epiregulin)属于表皮生长因子(EGF)家族,该家族成员通常通过结合表皮生长因子受体(EGFR/ERBB家族)调控细胞增殖、分化、迁移和存活等过程。EREG是一种分泌型蛋白,与EGFR(ERBB1)、ERBB4等受体结合后激活下游信号通路(如MAPK、PI3K-AKT),参与组织修复、炎症反应及胚胎发育。其功能特点包括促进上皮细胞增殖、血管生成及伤口愈合,在胃肠道、皮肤和乳腺等组织中表达较高。EREG的突变可能导致受体激活异常,例如功能获得性突变可能增强促增殖信号,与肿瘤发生相关(如结直肠癌、乳腺癌),而功能缺失突变可能影响组织再生。研究表明,EREG过表达会持续激活EGFR通路,促进肿瘤生长、转移及化疗耐药;在炎症性疾病(如类风湿关节炎)中,过表达的EREG可能加剧炎症反应。相反,EREG表达降低可能导致组织修复能力下降,例如在慢性伤口或黏膜损伤中恢复延迟。EREG与家族成员(如EGF、TGF-α、HB-EGF)的共性是均含EGF样结构域,通过ERBB受体传递信号,但各成员在受体偏好性、表达谱及功能细节上存在差异。例如,EREG对ERBB4的亲和力较高,而TGF-α主要靶向EGFR。EREG在肿瘤微环境中还可调控免疫细胞功能,如抑制T细胞活性,提示其作为免疫治疗潜在靶点。此外,EREG在心脏发育中通过ERBB4调节心肌细胞分化,其异常表达可能与先天性心脏病相关。
