BRCA1（乳腺癌1号基因，Breast Cancer 1）是一种重要的抑癌基因，属于BRCA基因家族，该家族还包括BRCA2。BRCA家族基因的共同特点是参与DNA损伤修复和维持基因组稳定性，尤其在双链DNA断裂的同源重组修复（HR）中起核心作用。BRCA1编码的蛋白质是一种多功能核蛋白，具有多个功能域，包括RING结构域（负责泛素化修饰）和BRCT结构域（参与蛋白质相互作用）。其主要生物学功能包括：1）DNA损伤修复，通过与RAD51等修复蛋白结合促进同源重组修复；2）细胞周期调控，在G2/M检查点发挥作用；3）转录调控，影响多种基因表达；4）染色质重塑。BRCA1主要作用位点是细胞核，尤其在DNA损伤位点聚集形成修复焦点。突变（如致病性截短突变）会导致蛋白质功能丧失，显著增加乳腺癌、卵巢癌、前列腺癌和胰腺癌等风险，这种癌症易感性被称为遗传性乳腺癌-卵巢癌综合征（HBOC）。BRCA1突变还可能导致对PARP抑制剂敏感，这是基于"合成致死"原理的靶向治疗策略。过表达BRCA1可能增强基因组稳定性但可能干扰正常细胞周期，而表达降低则会导致DNA修复缺陷、基因组不稳定性增加和肿瘤发生风险升高。BRCA1与多个信号通路相互作用，包括p53、ATM/ATR和Fanconi贫血通路，其功能异常会影响这些通路的活动。在临床应用中，BRCA1突变检测已成为遗传性癌症风险评估的重要指标，并指导预防性措施和治疗选择。需要注意的是，不同种群中BRCA1突变谱存在差异，某些突变（如Ashkenazi犹太人中的185delAG和5382insC）具有族群特异性高频特征。
